This chiropractic practice collects information from you (and your child where relevant) for the primary purpose of providing quality health care. We require you to provide us with your personal details and/or your child’s personal details and full medical history so that we may properly assess, diagnose, treat and be proactive in you or your child’s health care needs. This means we will use the information you provide in the following ways:
- Administrative purposes in running our chiropractic practice.
- Billing purposes, including compliance with Health Insurance and Medicare requirements.
- Use within the practice if discussing or passing your case to another practitioner within the practice for your ongoing management
- Disclosure to others involved in your health care or your child’s health care, including other chiropractors, doctors or specialists outside this chiropractic practice. This may occur through referral to other doctors, or for imaging and in the reports or results returned to us following the referrals.
You are not obliged to provide any of the information requested of you, but your failure to do so may compromise the quality of the health care and treatment given to you or your child.
You have the right to access the information collected about yourself or your child except in some circumstances where access might be subject to certain legal exceptions, which are set out in the Australian Privacy Principle 12 “Access to Personal Information”.
If your information is to be used for any other purpose other than set out above, your consent will be obtained.
